In order to earn more ortho nickels, the patients in practice must visit Dr. Izadi's website, and answer any two of the five trivia question listed here. Once you have any of the two answers, please send him an e-mail using the contact us icon with your name, phone number, the questions with answers, and the office you are going to. Dr. Izadi will give you 1 extra ortho nickel for each right answer, so you can earn two more nickels per month. You may answer only two of the questions per month, and the same questions can not be repeated the following months. If you answer more than two questions each time, it will not be accepted. Questions will change periodically, so you may check each month for more questions that you have never answered.
